Club MemorABLE Glow: A go-to Community for Social Connection and Engagement
Taking our good practices beyond the Lodge, we aim to establish a visionary and inspiring model of community care, building an organic culture of Person-Centred Care (PCC) everywhere. Club MemorABLE Glow, our community care model, is an innovative integrated social prescription model that provides opportunities for persons with dementia (PWDs) and mild cognitive impairments (MCI) as well as seniors in the community to unleash their potential and build on their strengths and abilities.
To empower each individual to live in wellness through active pursuits of new experiences, we also invite CM Glow members and their caregivers to seize opportunities for growth and continuous learning at our Chan Kum Leong (CKL) Club as participants, volunteers or as co-facilitators of various activities; exercise, music, art and mindfulness. Therapy Through Work (TTW)
Empowering persons with young onset and early-stage dementia, as well as persons with mild cognitive impairments (MCI) to continue meaningful employment through therapeutic work activities, with opportunities for socialisation, cognitive stimulation and continuous learning as they recultivate purposeful living.
