Associate Psychologist
Teaming with nursing staff, Therapists, Psychologists and Therapeutic Programme Executives in knowledge management to enhance the evidence-based practices, on evidence-based person-centred care and development of the integrated dementia care model to reframe dementia
Responsibilities:
- Managing assessments and evaluation of integrated approaches for Persons with Mild Cognitive Impairment (PMCIs) and Persons with Dementia (PWDs) of varying potential and abilities to live in wellness
- Provide leadership in strength-based Living Well Planning
- Provide leadership in psycho-social care and unique models of care for different profile of PWDs
- Lead multi-disciplinary team
- Support in review and enhancements of care planning, activity and harnessing environment to empower PWDs and PMCIs in the community to live in wellness
- Synergise with knowledge partners and co-creators for on-going transformation in integrated person-centre care
- Co-create knowledge from everyday best through documentation of good practices, build tested ground-up models for publication and sharing at conferences
Requirements
- A Bachelor Degree in Psychology with Honours
- Excellence in conceptualisation, writing and planning strategically leading to quality implementation, sense-making of data and documentation
- Experience with dementia population will be an added advantage
- Strong believer in person-centred care
- Dynamic, resourceful, driven, visionary and able to work independently and a great team player
- Excellent interpersonal skills
