Apex Harmony Lodge's Admission Philosophy

Apex Harmony Lodge believes that every elderly deserves quality person-centred care irrespective of their socio-economic status. Almost 90% of the Residents are subsidised by the Government, and Apex Harmony Lodge also supports residents in need through funds raised from donation drives and donors contribution.

Apex Harmony Lodge’s Admission Process

In order for us to understand your loved one and to provide better long-term care, a pre-admission assessment will be conducted at Apex Harmony Lodge. This assessment aims at gaining a holistic understanding of your loved one as an individual, including his/ her unique life story and needs.

Criteria for Admission

• Applicants must be diagnosed with dementia
• Applicants must be free from infectious lung diseases
• Applicants must be either ambulant or wheelchair-bound
• Both subsidised and private applicants must be Singaporeans or Singapore Permanent Residents

How to apply for subsidised residential care:

To apply, Person with Dementia shall be referred by Agency of Integrated Care, and the applicants’ immediate family members are to declare their income for Means Testing

• Family shall make appointment with the Medical Social Worker (MSW) of a restructured hospital or the polyclinic if they do not have an existing appointment with a restructured hospital.
• The MSW submits the referral to Agency for Integrated Care (AIC) where applicable.
• AIC will then refer the applicant to the appropriate Nursing Home.
